To schedule an appointment at Southeast, please give us a call at (415) 671-7000. Our hours are 8AM-5PM M-F for general care, and 7AM-7PM every day for urgent care.

To register as a new patient at our clinic, please call 415-671-7000 or come to our clinic to book a new patient appointment. Please come 30min before your appointment and bring identification & your insurance card (acceptable ID includes California ID, driver’s license and passport). Our registration staff will help guide you through the process of registering. 

*Due to the ongoing pandemic, we are scheduling new patients first as a telephone visit, followed by in-person visits as needed*

Sliding scale, Medi-Cal, Medi-Care and some private insurance are acceptable forms of payments for services. We can adjust our fees for people without insurance based on income and family size.

MyChart gives you online access to your health record. As a member of SF Health Network (SFHN) you are connected to top-rated primary, urgent, specialty, and emergency care services, and now to MyChart! The Southeast Field Care Clinic is part of the City of San Francisco’s strategy to preserve emergency room and hospital capacity while providing the best health care to all communities.
