Acupuncture

This age old healing practice has helped many patients alleviate pain and improve health, especially those with chronic pain or who are searching for an alternative to pain medication.

Diabetic Eye Screening

Diabetes can damage your eyes but annual eye exams can help detect damage early. We can now perform these exams on site so you do not have to go to the hospital.

Dental

We offer routine dental services for our community including tooth extractions and referrals for more complex procedures.

Food Pharmacy

We host a “food pharmacy” every other Thursday where we distribute fruits, vegetables, and sometimes meat along with cooking demonstrations, tips for healthy eating, and recipes.

HIV Services

We have a specialist care team trained to in treating patients with HIV. In addition, we have a PrEP advisor who is able to refer patients for preventative care.

Insurance Specialist

We know insurance can get complicated so we have several insurance specialists here on site to help you make informed decisions about insurance and access the care you need. We’re committed to providing affordable services for all.

Project Open Hand

Project Open Hand is a nonprofit organization that provides nutritious meals to seniors and the critically ill. To learn more about their food delivery van at Southeast every Tuesday, please ask your provider.

Mammogram Van

Mammograms help detect breast cancer and can save lives.  Call to ask when the mobile mammogram van will be here next.

Nutrition Services

Eating a well-balanced diet is very important but can also be hard. Our nutritionist would love to talk to you about your diet and how to improve what you eat. Our team can also help connect you with food resources.

Pharmacy Services

Not sure what medication you should be taking or how to take them? Want to make sure your medicine is working? Our in house pharmacist can help.

For eligible women and their children, our WIC office can help make WIC paperwork and benefits easy as well as provide access to many different resources to help live a happy and healthy life.

Our team of social workers can help connect you with housing, food, transportation, and other essential services.

Complex Care Management

For our high-risk patients with complex medical needs, we have team trained in managing a number of medical conditions and reducing the need for hospital care.

Medication-Assisted Treatment

We have providers who are trained to prescribe medications to treat drug addictions, and offer additional resources to those ready for the next steps.
